(b)(4).The bmw guide wire mentioned is being filed under a separate manufacturer report number.The device is expected to be returned for investigation.It has not yet been received.A follow-up report will be submitted with all additional relevant information.
 
Event Description
It was reported that the procedure was to treat an unspecified coronary artery.A balance middleweight (bmw) guide wire was advanced to the lesion and during advancement of a 2.0 x 6 mm otw mini trek the two devices froze together.The catheter could not advance further or be removed from the guide wire so the two devices were removed together and a new guide wire and dilatation catheter were successfully used to complete the procedure.There was no clinically significant delay in procedure and no adverse patient effects.No additional information was provided.

(b)(4).Evaluation summary: a visual, dimensional, and functional inspection was performed on the returned device.The reported difficulty removing the guide wire was able to be confirmed.The difficult to position was unable to be replicated in a testing environment due to the condition of the returned device.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no similar incidents from this lot.Based on the information reviewed, there is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.The investigation was unable to determine a conclusive cause for the reported difficulties.
