(b)(4).The reported complaint was confirmed.Manufacturer technical support troubleshooting provided the following solution with the biomed.The biomed reseated the computer card and the reported issue was resolved.The unit was returned to clinical use.Per the biomed: they do their own maintenance every three months, the water was not cloudy or discolored and he had no knowledge of patient infection associated with the cooler heater.No parts will be returned to the manufacturer for evaluation.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.

It was reported that during set-up of the device for a cardiopulmonary bypass procedure, the light emitting diode (led) lights did not come on main display when started.Start-up alarm worked as it should.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.
