Manufacturer Narrative

The actual device was returned for evaluation.Reliability engineering evaluated the device and the reported condition of the device overheating to the point that the tip of the device turned black was confirmed. It was determined that the shaft on the device was found to be discolored at the tip.It was further determined that the device would not accept the cutter; and that the function gauge could not be inserted. The assignable root cause was determined to be due to normal wear from normal use over time.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
