(b)(4).The actual device has been returned and is currently pending evaluation.Once reliability engineering evaluates the device, a supplemental medwatch report will be sent accordingly.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
 
Event Description
It was reported that during a maxillofacial surgical procedure, it was discovered that the attachment device had overheated to the point where the tip turned black.There was a couple of minutes delay to the surgical procedure.A spare device was available for use.The surgery was completed successfully.It was reported that no one was injured or burned.There was patient involvement reported.There were no reports of injuries, medical intervention, or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
 
Manufacturer Narrative
The actual device was returned for evaluation.Reliability engineering evaluated the device and the reported condition of the device overheating to the point that the tip of the device turned black was confirmed.It was determined that the shaft on the device was found to be discolored at the tip.It was further determined that the device would not accept the cutter; and that the function gauge could not be inserted.The assignable root cause was determined to be due to normal wear from normal use over time.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
