Event Description
|
It was reported that during an unspecified surgical procedure of the pelvis, it was observed that the small battery drive device had intermittent power.
It was noted that the device was more off than on.
It was reported that there was no delay in the procedure as an identical spare device was available for use.
It was reported that the the surgery was completed successfully.
There were no patient or user injuries reported.
It was reported there was no medical intervention or prolonged hospitalization.
All available information has been disclosed.
If additional information should become available, a supplemental medwatch report will be submitted accordingly.

The actual device was returned for evaluation.
Reliability engineering evaluated the device and the reported condition of intermittent power was confirmed.
An assessment was performed on the device which determined the unit was working intermittently.
It was also noted that one safety disc was missing, one insulation sleeve started to come out, and the electric motor was damaged (noise).
The assignable root cause was determined to be due to normal wear from normal use and servicing over time.
If additional information should become available, a supplemental medwatch report will be submitted accordingly.
