Event Description
It was reported that the nc trek was on the guide wire and was being loaded into the guiding catheter when resistance was noted and the mid-shaft separated.The device was removed without reported issue.There was no reported adverse patient effect or a clinically significant delay in the procedure.No additional information was provided.
 
Manufacturer Narrative
(b)(4).Evaluation summary: visual and dimensional analysis were performed on the returned device.The shaft separation was confirmed; however, the difficult to position could not be replicated in a testing environment due to the condition of the returned device.A review of the lot history record revealed no manufacturing nonconformities that would have contributed to this complaint.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation was unable to determine a conclusive cause for the reported difficulty to position; however, the shaft separation appears to be related to circumstances of the procedure.Based on the information reviewed, there is no indication of a product quality issue with respect to the design, manufacture or labeling of the device.
 
Event Description
Return device analysis revealed that the shaft separation was proximal shaft not mid-shaft.
