Event Description
It was reported that the asymptomatic patient recently had a normal device change out procedure.The leads were suspected of inserted into the wrong header resulting right ventricular lead in the left ventricular port and the left ventricular lead into the right ventricle port.On (b)(6) 2016, the leads were switched into the correct ports of the pulse generator.The patient experienced no adverse consequences.
