A field service engineer was at the customer site on 09/02/2016.The fse observed that another fse that had been at the customer site on 09/01/2016 had intentionally disabled the short sample sensor alarm on the instrument and instructed the customer to run without it.He escalated his observation to the field service district manager.The information was shared with the other fses in the district that disabling the sensor is not acceptable to make sure everyone was aware.The first fse is no longer with the company.Approximately 50 samples had been run with the sensor disabled and were repeated with no reports of erroneous results.The second fse ordered a short sample sensor and replaced the part on 09/07/2016.Per service request (b)(4) instrument was not operational for those five days (09/02/2016-09/07/2016).(b)(4).
 
Event Description
The field service engineer (fse) was at the customer site performing preventive maintenance on their ichem velocity automated urine chemistry system and disabled the short sample sensor on the instrument.Erroneous patient results were not generated and there was no change or effect to patient treatment in connection to the event.
