Event Description

It was reported from (b)(6) that during sterile processing, it was observed that the small battery drive device would stay engaged even after the trigger was released and the device was turned off.According to the reporter, the only way to stop the device was to remove the battery device.There were no delays to a surgical procedure.It was unknown if a spare device was available for use.There was no patient involvement reported.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

The actual device was returned for evaluation.Reliability engineering evaluated the device and observed that the device failed the start stop test, stays running on its own, the coupling head worn, there was an unknown residue/substance found on the internal components, the electric motor emitted an unusual noise and the ball bearings seized.Therefore, the reported condition was confirmed.The assignable root cause was determined to be due to improper cleaning/care and component wear from use and servicing over time.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
