The site declined to provide patient information, per (b)(6) privacy laws.A replacement computer was sent to the site, however, was not needed and returned to manufacturer, unused.- a medtronic representative performed a navigation system check-out, hardware and instruments areas passed.Software test failed.Successfully launched the software without any issues.Both re-boot and launched and exited the application several times and all works fine.Found the issue was due to patient database which showed excessive 296 patients.Informed the site to clean the patient database every three months to avoid repeated issues.Completed full system check-out and all tests passed successfully.Navigation system is fully functional and ready for clinical use.- no further issues have been reported.
 
Event Description
A site representative reported that, while in a spinal fusion procedure, they booted their navigation system and it was displaying a black window with text in it.The navigation system then re-booted twice and the system exited again.A second navigation system was brought in to be used to continue the procedure.No further details regarding this issue, or specifically when it occurred, were provided.The surgeon opted to continue and completed the procedure with the use of the navigation system.Delay in therapy was less than one hour.There was no impact on patient outcome.
