Age at time of event: 18 years or older.Device evaluated by mfr: the device was not received for analysis.The manufacturing batch record review confirmed that the device met all material, assembly and performance specifications.The most probable root cause is considered handling damage as the event occurred without direct patient contact.(b)(4).
 
Event Description
It was reported that product sterility was compromised.A 3.5mm x 40mm x 146cm coyote¿ es balloon catheter was selected to treat the lesion.However upon pulling out the balloon that was tightly coiled from the hoop, the device "jumped off" or fell off from the table.The balloon was discarded after being contaminated and the procedure was completed with another of the same device.No patient complications were reported.
