Event Description
Information was received from a healthcare professional via a manufacturer's representative regarding a patient who was receiving 10 00mcg/ml gablofen at a dose of 349mcg/day via an implantable infusion pump for an unknown indication for use.It was reported that the patient had experienced withdrawal and the pump had alarmed.When removing the medication from the pump 41cc's was removed which would suggest overfilling.The pump logs were read but the results were unknown.The pump was replaced.It was reported that the issue was resolved at the time of the report.The patient status was "alive-no injury." no further complications were anticipated/reported.

Event Description
Additional information was received from a manufacturer's representative.It was reported that when the pump was interrogated it was clear that a motor stall had occurred.No further complications were anticipated/reported.
