Event Description
It was reported that the balloon covering was not removed during preparation and was accidentally left inside the patient's body.A coyote pta balloon dilatation catheter was advanced inside the patient without removing the balloon protector.The balloon protector dislodged inside the patient and had to be retrieved at a different time.No further patient complications were reported.
