Event Description
It was reported that the procedure was to treat a lesion that was previously stented located in the right common iliac vein.The armada 35 12 mm / 20 mm balloon catheter ruptured during the first inflation at 10 atmospheres.A piece of the balloon separated where the marker was but stuck to the distal end of the guide wire and was easily removed through the sheath.The instructions for use were followed with no issues noted during preparation.There was no resistance during advancement or withdrawal of the device.There was no reported adverse patient effect.There was no reported clinically significant delay in the procedure.No additional information was provided.
 
Manufacturer Narrative
(b)(4).The device was not returned for evaluation.A review of the lot history record revealed no manufacturing nonconformities that would have contributed to this complaint.Additionally, a review of the complaint history did not indicate a lot specific quality issue.The investigation was unable to determine a conclusive cause for the reported difficulties.Based on the information reviewed, there is no indication of a product quality issue with respect to manufacture, design or labeling.
