Event Description
|
Boston scientific received information that this cardiac resynchronization therapy pacemaker (crt-p) exhibited right ventricular (rv) impedance measurements of less than 200 ohms and greater than 3000 ohms.The threshold measurements were also increased.The device also appeared to be prematurely depleting, and the physician thought there was a possible set screw issue with the rv lead.A surgical revision was performed and the patient's rv lead was tested via the pacing system analyzer (psa).The high impedances were noted with the psa as well.The device was explanted and replaced.No additional adverse patient effects were reported.

Upon receipt at our post market quality assurance laboratory, a thorough evaluation of the product was performed.Review of device memory confirmed the presence of a higher than normal current drain condition.Electrical testing and analysis were then conducted, which isolated the high current to an anomaly in an oxide layer within a custom integrated circuit component.This anomaly caused a high current drain, which over time resulted in the reported clinical observations.
