Event Description
|
A biomedical engineer (biomed tech) stated in setup mode a burning smelled coming from the deaeration motor.The biomed tech noticed that the deaeration motor and wiring harness were both burned and the machine was operation and displayed a flow error.Additional follow-up revealed the issue was detected during setup and confirmed there was no patient involvement or patient injury.The biomed tech stated no visible smoke, flames or sparks were observed.The biomed tech confirmed the deaeration motor and wiring harness components appeared burned and stated replacement components were ordered and pending replacement.The biomed tech stated the machine remained out of service, and once repaired and is qc tested the machine was to be placed back in service.

The device was not returned to the manufacturer for physical evaluation and the failure mode cannot be confirmed.However, an investigation of the device manufacturing records was conducted by the manufacturer.There were no deviations or non-conformance during the manufacturing process.All device history records (dhr) are reviewed and released according to the dhr review checklist and release procedure.A device is not released if it does not meet requirements or is nonconforming.In addition, the device record review confirmed the labeling, material, and process controls were within specification.
