Event Description
The user facility reported during a ureteroscopy procedure, after multiple use throughout the procedure, the ncircle delta wire tipless stone extractor would no longer retract entirely.The procedure was able to be completed with the device.No unintended portion of the device remained inside the patient¿s body.No additional procedures were required due to this occurrence.No adverse effects or consequences were reported.
 
Manufacturer Narrative
A review of the manufacturing instructions, quality control, specifications, drawing, functional test and visual inspection of the actual device was completed during the investigation.One device was returned for investigation.The collet knob is tight and secure.The mlla (male luer lock adaptor) is loose.The pett measures 3 cm in length.A functional test determined the handle does not actuate the basket formation.The support sheath and basket sheath are still adhered.A kink was noted in the basket sheath at 85 cm from the distal tip.A visual examination noted the support sheath is bowed in appearance.After manually checking the basket sheath for kinks, damage, and anomalies, the handle actuated the basket formation.The handle was reset and reassembled and the basket actuated fine.No patient harm was reported.There is no indication that a design or process related failure mode contributed to this event.Review of production and quality documentation did not observe any specific issues with current manufacturing or quality controls that may have contributed to this incident.Based on the investigation evaluation, there is no indication that a design or process related failure mode contributed to this event.Current controls for manufacturing are in place to assure functionality; device integrity prior to shipping.Based on the provided information a definitive root cause cannot be established or reported at this time.Appropriate personnel have been notified to monitor for similar complaints.Per a quality engineering risk assessment no further action is required.
