Event Description
|
This complaint has been evaluated based on the information provided; there is no allegation of death or serious injury.The customer reported that the patient is unable to be heard in the ct control room through the gantry microphone.If the operator is unable to hear the patient due to a failed microphone, there is potential for injury to the patient.This issue has been determined to be a reportable event.

A philips field service engineer (fse) reported that while onsite (b)(6) 2017 performing a planned maintenance (pm), the customer reported that a patient was unable to be heard in the ct control room through the gantry microphone.The customer did not report the issue to philips prior to the pm.The customer confirmed with the fse that all patients in the scan room were being visually supervised by the operator from the ct control room.Therefore, there was no patient impact and no harm as a result of this issue.The fse evaluated the system and found that the front gantry breathing light assembly failed.The fse replaced the front gantry breathing light assembly to correct and resolve the issue.The system is operational and in clinical use.This event has been determined not to be a reportable event.
