Event Description
|
It was reported from (b)(6) that during an unspecified surgical procedure, it was observed that the oscillating saw attachment device stopped working and noise was coming from the device when the trigger was pressed.It was further reported that the battery device was changed; however, the issue still persisted.It was unknown if there was a delay to the surgical procedure.The procedure was completed with a spare device.There was patient involvement.There were no injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch will be submitted accordingly.

This device was returned for service; however, did not meet manufacturing specifications during pre-repair assessment.During repair, it was determined that the reported condition was confirmed.It was determined that the coupling tool side was not functioning and defective.It was further determined that the tool holder was defective.It was further determined that the device failed pretest for check the machine coupling and check symmetry with handpiece.The assignable root cause was determined to be due to improper maintenance, which is user error, misuse, and / or abuse.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
