(b)(4) there were multiple reports submitted for this event: mfr# 0001825034-2018-00291.Concomitant medical products: comprehensive reverse e1 humeral bearing pnep-115393 ln320400.Customer has indicated that the product is in process of being returned to zimmer biomet for investigation.Once the investigation has been completed, a follow-up mdr will be submitted.

Event Description
|
It was reported that the patient underwent a reverse total shoulder arthroplasty.During the procedure, it was noticed that the locking ring would not hold the poly as it appeared to be bent.The deformation was observed as the surgeon was putting the poly into the humeral tray.The surgery was completed using another humeral tray.There was no surgical delay.No patient impact was reported.

This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event was confirmed.Visual inspection revealed ringloc was bent within the tray.Visual inspection of the humeral tray indicates it is conforming to the print, except for the bend.There are no burrs or obstructions in the tray¿s ringloc groove.Dimensional inspections reveals measurements are within specifications.Device history record (dhr) was reviewed and no discrepancies were found relevant to the reported event.Root cause of the event could not be determined.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
