Event Description
It was reported that an amia automated pd cycler set leaked.The patient was not connected at the time of this event.The patient walked over to the amia cycler to connect and there was a large puddle on the ground under their amia cycler and there was also fluid dripping from the cassette chamber.Apparently one of the tubes had detached somehow from the cassette.There was no patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
The actual device was not available; however, a photograph of the sample was provided for evaluation.A batch review was conducted and there were no deviations found related to the reported condition during the manufacture of this lot.A visual inspection of the photograph was performed with the naked eye.Visual inspection identified the heater bag tubing was separated from the cassette on one photograph which is indicative of a leak.The reported condition was verified.An assignable cause could not be determined.Should additional relevant information become available, a supplemental report will be submitted.
