(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.The device is expected to be returned for evaluation.It has not yet been received.A follow up report will be submitted with all relevant information.
 
Event Description
It was reported that when the 1.5 x 15 mm mini trek balloon catheter was removed from the dispenser coil the hub was observed to be separated from the shaft.The device could not be used on the patient.A new mini trek balloon catheter was used to complete the case successfully.There was no clinically significant delay in the procedure reported.No additional information was provided.
 
Manufacturer Narrative
(b)(4).The device was returned to abbott vascular (av) for analysis and the hub separation was confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no similar incidents reported from this lot.Av conducted root cause analysis and determined the reported separation appears to be related to a potential product quality issue.The issue is being addressed per internal operating procedures.Av will continue to trend the performance of these devices.
