Serial number is unknown.This information will be provided in a supplemental report if made available.As the serial number is unknown, the device manufacture date could not be determined.This information will be provided in a supplemental report if made available.Livanova (b)(4)manufactures the centrifugal pump 5(cp5).The incident occurred in (b)(6).A livanova field service representative was dispatched to the facility to investigate.The service representative was able to confirm the reported issue and traced the failure to a incorrect calibration of the device.The technician performed a zero calibration to resolve the reported malfunction.Subsequent functional verification testing was completed without further issues and the unit was returned to service.It was not possible to determine why the values were out of specification.A review of the dhr did not identify any deviations or non-conformities relevant to the reported issue.If any additional information pertinent to the reported event is received, it will be provided in a supplemental report.Device evaulated by livanova rep.
 
Event Description
Livanova (b)(4) received a report that a centrifugal pump 5 (cp5) could not be used properly during procedure.The customer reported that if the knob of the drive unit was used to adjust the speed the unit worked properly but if the knob was released the drive unit stopped.There was no report of patient injury.
