Event Description
It was reported that during start up, the ventilator touchscreen freezes.It was reported that the device was in clinical use at the time of the event.There was no patient harm reported.
 
Manufacturer Narrative
Date of manufacturer: 07/29/2018.Date of report: 09/22/2018.(b)(4).The field service engineer (fse) replaced the touchscreen assembly to address the issue.The ventilator passed all testing and operated within the manufacturing specifications.Failure analysis on the returned touch screen shows that physical damaged resulted in the scratches on the screen.
