A medtronic representative went to the site to test the equipment.Testing revealed that replacing surgeon monitor, monitor kit and monitor cable resolved the issue.The system then passed the system checkout and was found to be fully functional.The parts have been received by the manufacturer for evaluation.The surgeon monitor was returned to the manufacturer for analysis.Analysis found that the bezel was pulled apart along the top edge.When connected to a good known system, the monitor powered on without any issue and displayed an image.However, after 30 minutes, the display started flickering.Analysis found that the reported event was related to a electrical issue.The hardware investigation found that the reported event was related to a hardware issue.This issue was documented in a medtronic navigation hardware anomaly tracking database.The monitor kit was returned to the manufacturer for analysis.Analysis found that when connected to a good known system the monitor displayed a normal image at power up but went back after some time.Analysis found that the reported event was related to a electrical issue.The hardware investigation found that the reported event was related to a hardware issue.This issue was documented in a medtronic navigation hardware anomaly tracking database.The monitor cable was returned to the manufacturer for analysis.Analysis found that the cable was in good condition with no apparent physical damage.Cable passed continuity test with no open or shorts.No fault found.
 
Event Description
Medtronic received information regarding a navigation device being used intra/peri-operatively of a functional endoscopic sinus surgery (fess) procedure.It was reported that the surgeon monitor of the navigation system was constantly flickering from the early stages of an ear, nose & throat (ent) case.The staff cart was not flickering.The screen issues continued with or without an instrument plugged in.The issue was resolved when the site bypassed the video splitter.There was no delay to procedure.There was no known impact on patient outcome.
