(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.The stent remains in the patient.Investigation is not yet complete.A follow up report will be submitted with all relevant information.
 
Event Description
(b)(6).It was reported that the procedure was to treat a narrow left main and a narrow proximal left anterior descending (lad) artery that were both 85% stenosed.On 03/24/2014, the patient presented with paroxysmal precordial discomfort.Therefore, a 3.0 x 12 mm and a 4.0 x 15 mm xience xpedition stent were implanted in the left main and the proximal lad.The patient was discharged on (b)(6) 2014.On (b)(6) 2017, the patient was re-hospitalized for rapid heart beat and hypertension.An angiography and a computerized tomography were not performed.Medical treatment was provided to the patient, and they were discharged.The symptoms occasionally occur.According to the physician, the relationship between the event and the device is unknown.No additional information was provided.

(b)(4).The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot and relabeled lot that would have contributed to this event.The reported patient effect of hypertension is listed in instructions for use everolimus eluting coronary stent systems, xience xpedition as a known patient effect of coronary stenting procedures.A conclusive cause for the reported patient effect, and the relationship to the product, if any, cannot be determined.There is no indication of a product quality issue with respect to manufacture, design or labeling.
 
Event Description
Subsequent to the initial report, additional information was received.The patient was re-hospitalized for atrial fibrillation.No additional information was provided.
