(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.The customer reported the device was discarded.Investigation is not yet complete.A follow up report will be submitted with all relevant information.
 
Event Description
This is filed to report the atrial septal defect, requiring treatment.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 3.One clip was implanted in the mitral valve, reducing the mr to 1.After the steerable guide catheter (sgc) was retracted from the left atrium over the septum to the right atrium, an intra-atrial bidirectional shunt was observed.The decision was made to close the asd with an asd device.The tricuspid valve was then treated successfully with one clip.There was no clinically significant delay in the procedure and no adverse patient effects.No additional information was provided.

(b)(4), patient selection for use in the tricuspid valve.The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot.Based on the information reviewed, the reported patient effect of atrial septal defect (asd) was likely related to procedural conditions.It should be noted that the mitraclip instructions for use (ifu) states that the mitraclip system is intended for reconstruction of the insufficient mitral valve.The off-label use did not likely influence the event.The reported patient effect of asd, as listed in the mitraclip system instructions for use, is a known possible complication associated with mitraclip procedures.There is no indication of a product quality issue with respect to manufacture, design or labeling.
