Event Description
Patient was having a egd with bravo placement.The bravo probe did not deploy on the first try.So, the physician had to try again it still did not deploy.The physician had to break the handle in order for the bravo to deploy.Manufacturer response for bravo ph capsule, (brand not provided) (per site reporter).The provider reached out to medtronic about this issue in the past, not sure he did again with this incident.
