Event Description
Information was received from a healthcare provider (hcp) via a company representative regarding a patient who was currently receiving baclofen with concentration 2000 mcg/ml at a dose rate of 12 mcg/day via an implantable pump for intractable spasticity and movement disorders.It was reported that an alarm was heard and confirmed via telemetry using a tablet/application (clinician programmer).Regarding the alarm, safe state occurred.The pump defaulted to a minimum rate mode.Prior to minimum rate, the patient was receiving baclofen at a dose rate of 909.4 mcg/day.No patient symptom was reported.No further patient complications have been reported as a result of this event.

Event Description
Additional information was received via a company representative.The critical pump alarm occurred on (b)(6) 2018.Event code was 07 was further noted regarding bolus settings must be completed prior to update.The last entry on the log was (b)(6) when the pump was refilled by the physician¿s office.The cause was undetermined.No further troubling shooting or diagnostics were performed.The pump was put back to the original dose.The company representative was not aware of any impact to the patient.At the time, the company representative had met with the patient and she was not experiencing a change in therapy.The patient did not know her current weight.The device remains in service.
