An alarm indicative of a potential malfunction of the disposable cassette was reported.The device was discarded and the lot number is unknown; therefore, a device analysis could not be completed.However, a leak on the supply line of the homechoice cassette was reported, which is known to cause this alarm.The cause of the leak could not be determined.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported that a homechoice device experienced a system error 2240 (air in line/set) alarm and a system error 2267 (air and fluid in set) alarm.The home patient (hp) was connected during an unspecified cycle at the time of the alarm.This occurred during peritoneal dialysis therapy.During follow up, it was verified that the leak was located on the supply line.There were no damage noticed on the supplies; no loose connection between the supply line and supply bag.Renal therapy services (rts) reviewed proper procedures with the patient.The hp would contact the nurse regarding missed therapy.There was no patient injury or medical intervention associated with this event.No additional information is available.
