Event Description
It was reported that the patient line of a homechoice cassette leaked; further described as ¿it was squirting out of the hole." this was observed during drain one of five of peritoneal dialysis therapy; the patient was connected.Renal therapy services (rts) assisted the patient in ending therapy and advised to start over with all new supplies.Rts advised to inform their peritoneal dialysis registered nurse (pdrn) regarding the event.There was no patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
Additional information: two actual (2) samples were received for evaluation.A visual inspection was performed with the naked eye noted a cuts in the tubing on both samples.One (1) sample had cuts in the tubing approximately 22 inches from the blue shrouder on the heater line.The other sample had a cut approximately 37 inches from the patient connector on the patient line.Functional testing including clear passage and clamp function testing were performed with no issues noted.Leak testing revealed a leak through the cuts of both samples.The reported condition was verified.The cause of the condition could not be determined.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
