The customer contacted physio-control to report that during a synchronized cardioversion procedure, the patient had a very complicated rhythm.The sync button was engaged but the device discharged on the "t" wave causing the patient to go into ventricular fibrillation.They were able to change the device to lead 2 and convert the patient out of that rhythm.There were no reports of adverse effects to the patient.Physio-control has made multiple attempts to contact the customer in order to obtain additional information about both the patient and the event; however, no response has been received.
