Date of event: (b)(6) 2018.Date of report: 09/16/2019.The manufacturer¿s international service technician confirmed both of these issues.The manufacturer¿s international service technician replaced the flow sensor assembly to address the airflow accuracy test.Power switch overlay was replaced to address the led illumination failure.The unit was checked overall, run in tested, cleaned and functionally tested and no abnormality was confirmed.The gas delivery system (gds) assembly was assembly was return for analysis.An investigation was performed and u1 on the air flow sensor pcba created the air and o2 flow accuracy test to fail.The power switch overlay was not returned to failure investigation; therefore, the root cause of the led illumination failure could not be determined.
 
Event Description
During preventative maintenance, (pm) the manufacturer¿s international service technician reported the device failed the airflow accuracy test (pvt test 5) at the 0slpm setting and that the green light emitting diode (led) of power switch had illumination failure.There was no patient involvement.
