Event Description
According to the reporter, after the catheter implantation, but prior to the patient's dialysis procedure, it was noted that the venous (blue) luer adapter had a crack (per photo attachment).The event was observed when the syringe was connected to the adapter (prior to the patient connection with the bloodlines/machine).Chlorhexidine and betadine solution was used as a cleaning agent on the device and it was mentioned that the site was not treated prior to product placement.It was reported that there was no leak and no tego was utilized.The catheter was said to have been repaired.There was no reported patient outcome.
