Event Description
A user facility biomedical technician reported that a fresenius 2008t machine will not power on after changing the power supply and had a blank display.The machine abounded a constant audible alarm.The 24-volt cable plugged in upside down at the power control board.Upon follow up, the biomedical technician reported that the nurse unplugged the machine during mid- treatment due to the outlet catching fire.The biomed stated that the machine was pulled out of the outlet on the wall and fire was observed.The biomed stated that the nurse observed smoke, however there was no evacuation.The biomed stated that the patient did not develop any symptoms, adverse events, injuries, or require medical intervention as a result of the reported event.The biomed stated that the outlet caught on fire and damaged parts of the fresenius 2008t machine.The machine was plugged into a hospital grade ground-fault circuit interrupter (gfci) outlet.The biomed stated that the cause of the fire was due electrical wiring in the outlet, not the fresenius 2008t machine.The motherboard and the power supply were replaced due to the damage from the fire in the outlet.The machine has not been returned to service.The biomed stated that the machine has approximately 100 hours and had been in use for two weeks.The damaged parts have been returned to the manufacturer for evaluation.
 
Manufacturer Narrative
Plant investigation: no parts were returned to the manufacturer for physical evaluation.Additionally, no on-site evaluation was performed by a fresenius regional equipment specialist (res).A records review was performed on the reported serial number.An investigation of the device manufacturing records was conducted by the manufacturer.There were no non-conformances, or any associated rework identified during the manufacturing process which could be related to the reported event.In addition, the device history record (dhr) review confirmed the results of the in-progress and final quality control (qc) testing met all requirements.The investigation into the cause of the reported problem was not able to be confirmed.A definitive conclusion regarding the complaint incident cannot be reached without a physical examination of the complaint device.
