Event Description
It was reported that during the implant attempt, when the lead entered the catheter, it was folded and a crease appeared.It was not able to enter the target vein and was not implanted.No patient complications have been reported as a result of this event.

Product event summary: the full lead was returned and analyzed.The analysis indicated that the distal conductor was extrinsically distorted.The proximal conductor of the lead became extrinsically distorted.The analyst noted that the conductors are distorted at 76.2 cm from the proximal end of the lead.Visual analysis of the lead indicated damage at implant.If information is provided in the future, a supplemental report will be issued.
