It was reported to boston scientific corporation that a stone cone nitinol retrieval coil was used in the ureter during a holmium laser lithotripsy procedure performed on (b)(6) 2019.According to the complainant, during the procedure, the coil was placed on the proximal tip of the stone to prevent the stone from moving up.However, during lithotripsy, the fiber contacted with the coil and broke the ptfe heat shrink.A forceps was used to remove the broken coil.Another stone cone was used to complete the procedure.There were no patient complications as a result of this event.
