Event Description
|
The patient was undergoing a coil embolization procedure in the anterior cerebral artery (aca) using penumbra smart coils (smart coils) and a non-penumbra microcatheter.During the procedure, the physician encountered resistance while advancing a smart coil to the target location and decided to retract the coil.On the third attempt, the physician encountered significant resistance and subsequently, the smart coil jumped back while the microcatheter shot forward and perforated the aneurysm wall.The smart coil was then completely removed.The procedure was completed using non-penumbra coils with the same microcatheter to stop the bleed and thrombose the remainder of the vessel.The patient was stable and was set to discharge home on (b)(6) 2019.
