A steris service technician arrived onsite following the reported event to inspect the evolution transfer carriage and found the lock pin springs were not operating properly. The lock pins are located on the bottom of legs of the transfer carriage and allow the wheels to lock into place. The root cause of the reported event can be attributed to the springs within the lock pins. The springs had worn causing the lock pins not to stay in place and for the transfer carriage to retract towards the employee resulting in the reported event. While onsite, the technician replaced the springs, tested the unit, and found the transfer carriage to be operating according to specification. No additional issues have been reported.
