Medtronic received a report that the distal tip of the pipeline pushwire broke off inside the patient.The patient was undergoing surgery for treatment of a saccular, unruptured, right ic aneurysm with a max diameter of 12 mm and a 4.7 5x3.5 mm landing zone.It was noted the patient's vessel tortuosity was moderate.It was reported that after deploying the pipeline, the pushwire was attempted to be retracted into the phenom27 microcatheter, but r esistance was felt.The phenom was retrieved without being retracted.An angiogram confirmed the distal tip of the pushwire was left inside the body.A 4mm gooseneck snare was used to successfully recover the pushwire tip.There was no resistance or difficulty in the operation and the stent was successfully placed.Postoperative blood flow angiography showed no problems.It was indicated that all devices were prepared as per the instructions for use (ifu), and no patient symptoms or further complications were reported as a result of this event.Ancillary devices include an envoy6fr guide catheter, phenom 27 microcatheter, and a chikai014 guidewire.
