Description of event according to short form complaint filed: it is alleged that "[pt] received a cook celect filter on (b)(6) 2008.It is alleged that the [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.".
 
Event Description
Per a ct (computed tomography) scan of the abdomen dated (b)(6) 2019, ¿ivc filter as described with inferior positioning of the filter and multiple filter strut perforations as described.Minimal tilt.Moderate ivc stenosis.There is a stent seen in the left common iliac vein and extending into the inferior ivc.¿.

Patient allegedly received an implant via right jugular vein due to deep vein thrombosis (dvt).Patient is alleging migration, tilt, vena cava perforation and embedment.The patient further alleges depression and physical limitations.
