|
It was reported that an unspecified quantity of access sets were connected to an infusion pump and the pump displayed "upstream occlusion." this was observed during aminocaproic infusion.It was further reported that the upper injection port was "away enough," the bag was elevated, and the clamp was open.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

The actual device was not available; however, a photograph of the sample was provided for evaluation.A visual inspection was performed to the photograph using the naked eye which did not identify any abnormalities that could have contributed to the reported condition.The photograph shows a spectrum iq infusion pump with a upstream occlusion message in the display screen.There is a what appears to be a baxter solution set loaded into the pump.The reported condition is not clearly observed via the photograph and due to the nature of the sample no additional testing could be performed.Therefore, the reported problem could not be verified.Should additional relevant information become available, a supplemental report will be submitted.
