It is alleged that the patient received a gunther tulip on (b)(6) 2008.It is alleged that the patient was injured without further explanation.Hospital and medical records have been requested but not yet provided.

Patient further alleges "i'm unable to lift anything over xxxx pounds, am unable to walk and tire out very quickly.I've had several blood clots that have led to sepsis.My breathing is also very weak at times" and "i'm unable to lit weight, hike, kayak or canoe." the patient also alleges depression and anxiety.

Patient allegedly received an implant.Patient is alleging tilt, vena cava perforation and abutting of ivc wall.Report from computerized tomography (ct): "ivc filter surveillance." "an infrarenal ivc filter appears intact.Tilting of the filter with the apex abutting the right posterolateral ivc wall.Perforation of 6 to 7mm of the distal aspect of the left lateral strut into the aorta.An anterior strut perforates the ivc wall, and abuts mesenteric vasculature and prominent but nonenlarged aortocaval lymph node.A right lateral strut perforates the ivc wall and abuts the adjacent ascending colon, without appreciable intraluminal bowel perforation." "mild apex right posterolateral tibial of an intact infrarenal ivc filter.A strut perforated the aorta.".
