Event Description
It was reported that an occlusion alarm occurred. Customer experienced elevated blood glucose (bg) level ranging from 450 mg/dl to greater than 600 mg/dl, with high level of ketones. Parent administered fluids due to vomiting and health care provider prescribed anti-nausea medication. Customer reverted to manual insulin injections. Reportedly, vomiting lead to bruising and long term organ damage. A system check was performed with tandem technical support and root cause could not be determined.
