Describe the Event
or Problem:
ParaGard IUD placed in Women's Clinic (in place 6.5 years) Patient presented to clinic for removal of IUD. 3 "tugs" with ring forceps. IUD came out on third tug, missing both arms. X-ray ordered and patient given appointment for KUB and gyn clinic follow-up. Patient presented "removal of rest of IUD". Sonogram showed arms seen embedded into myometrium. Patient scheduled for hysteroscopy and removal of retained fragments of IUD
