The following electronic speeches, meetings, and intergovernmental invitations request questionnaire is for Office of the Commissioner requests only. Please see the Centers, Offices, and Programs pages for all other requests.
To complete your speaker request submission, you will need to do the following:

Set aside at least 15-minutes to enter all the speaker requests for each event during one session. DO NOT press "VERIFY" until you have verified data for each
numerated line. Afterwards, press "VERIFY". A confirmation dialogue box will display. Click "OK" if data is correct and complete.
Data may be revised by clicking "Cancel" on the confirmation dialogue box. Press "OK" to advance through the confirmation dialogue box pages.
Click "OK" if data is correct and complete then information will be officially transmitted for processing. DO NOT press the back arrow button in your browser as
this may cause the information to not be submitted in its entirety.
It may take 15-minutes to fill out the questionnaire initially during each session, but subsequent requests will pre-populate the fields making it quicker to submit the entire request .
Microsoft Edge is the only officially supported browser for FDA. Other browsers (Chrome, Firefox, Safari, etc.) have not been verified and may prevent successful form submissions.
