If you are requesting your own personal or organizational records in unredacted (first party) format, you must indicate that in the description of your request and provide first party documentation.

If you are seeking personal or organizational records in unredacted (first party) format about another entity, you must attach a signed authorization from the person or organization whose information you are seeking.

This form is for submission of FOIA Request by members of the public. Do not use this form to submit Consumer Complaints.
|
|
Requests for Consumer Complaint status updates cannot be processed under FOIA and should be made to the Consumer Complaint Coordinator (CCC) directly. CCC are listed by state.

All Required (*) fields must be completed before submitting your request.

A valid email address is required.
|
|
A complete description of your request must be in the Subject field.
|
|
An electronic copy of your Request letter may be attached to the Request form.
|
|
Please include as much detail as possible about the specific records you are requesting.

Multiple requests may be entered while you are logged in.
|
|
After submitting a request you will be given a confirmation # and the ability to print a copy of your request.
|
|
After acceptance by FDA FOI staff an electronic Acknowledgement letter with your FOI Control # will be sent to the email address provided in the Request.
