The production device history record (dhr) for this intra-aortic balloon pump (iabp) is not required to be reviewed per getinge standard operating procedure since the iabp was manufactured more than a year before the date of event.A getinge field service engineer (fse) evaluated the iabp unit and found that the faceplate was protruding away from the iabp unit, making it impossible to make the connection to the fiber optic connector.The fse took the faceplate off and put it back on correctly, then the fse was able to test the iabp unit with his fiber optic tester.As a result, the fiber optic function was working properly.The fse mentioned that the last repair on this iabp unit was for a broken fiber optic connector, and that this appeared to be a pattern and abuse of unit.The fse then performed all functional and safety checks to meet factory specifications.Unit passed all functional and safety test per factory specifications.The iabp was then released to the customer and cleared for clinical service.(b)(6).
