The freedom driver will be returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The customer, a syncardia authorized distributor, reported that the freedom driver exhibited a fault alarm while supporting the patient.There was no reported adverse patient impact.The customer also reported that the patient was subsequently switched to a backup driver.
 
Manufacturer Narrative
Visual inspection of the driver revealed the secondary motor's cam follower out of the bottom dead center (bdc) position.When the primary motor was removed for further visual inspection, accumulated grease was observed and rotating the motor by hand was difficult.During investigation testing, the customer-reported irreversible fault alarm was reproduced when the driver started up in secondary motor operation and alarmed as designed.The root cause of the of the customer-reported alarm was the engagement of the secondary motor as a result of increased resistance on the primary motor circuit as evidenced by the observed grime and difficulty in rotation.This issue will be monitored and trended as part of the customer experience process.Syncardia has completed its investigation and is closing this file.Ce 5280 follow-up report 1.
