This is filed to report thrombus. It was reported that this was a mitraclip procedure to treat mixed mitral regurgitation (mr) with a grade of 4.During insertion of the steerable guiding catheter (sgc) and dilator, when the dilator tip started to cross the septum, thrombus was observed on the tip of the sgc.The activated clotting time (act) was at 276 seconds, so additional heparin was administered. The sgc and dilator were removed and the thrombus disappeared in the right atrium. He devices were re-flushed and were used without further issues. Two clips were implanted, reducing mr to 1+.There was no adverse patient sequela. No additional information was provided.
