Event Description
|
On (b)(6) 2020, an avr was performed due to severe aortic regurgitation on the patient.After measuring the size with an abbott sizer(tf2000), the surgeon selected a 23mm trifecta gt valve; the trifecta gt valve was implanted with everting mattress suture technique using pledgets.Before closing the patient's chest, the aortic root became torn and regurgitation occurred on the patient.The trifecta gt valve was explanted and the aortic root was repaired.After that, a 21mm trifecta gt valve was implanted in the patient with no adverse consequence on the patient.The patient is in stable condition postoperatively.Other patient information: height: 150cm, bsa(m2): 1.31 m2.

Additional information for g4, g7, h2, h3, h6, and h10.An event of the aortic root of the patient tearing and regurgitation was reported.The investigation confirmed the device met dimensional specifications when analyzed at abbott.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications.The cause of the reported event could not be conclusively determined.
